What to Bring to Your First Consultation

Please arrive 10 minutes before your appointment time to complete your new patient forms and pain assessment documents. 

Please remember to bring the following to your first appointment: 

  • Current x-rays and scans. If you're unable to obtain copies of these, please call our office and we'll help you to source these. Without them an additional appointment may be required to accurately assess your condition.

  • A current referral from your GP or specialist. This is necessary for you to receive partial reimbursement from Medicare for your appointment.

  • Medicare, DVA, Private Health Insurance or Work Cover details if applicable.

Cost

Part of your initial consultation fee will be eligible for a Medicate rebate. You'll need to make the full payment, but we're happy to process the Medicare claim for you.

Support Person

Many people find it helps to bring a support person with them – your partner or a friend. You're very welcome to bring then with you.  

What can I expect?

Dr Slater allocates 30 minutes for your initial consultation. However, each case is different and will depend on your condition. If a surgical procedure is recommended, your appointment may extend to 60 minutes. While he will discuss all of your treatment options, the choice to proceed with a surgical procedure is entirely yours.
